If you can upload your form to your website like: yourwebsite.com/contact-form
Then you can use that URL to embed that in Notion.

Ah, thanks for letting me know.
As I'm not using a server to host a website, and rely on Notion + Styled.page to present content, I'm not able to host the form.
If that is something that can be arranged and/or an integration with Notion can be added, please let me know and I'll be more than happy to jump on board. Thanks!
