Not Very Professional
It works, but it doesn't look very professional. The form looks childish with big goofy icons. Also, whats with the 'REPORT ABUSE" link? When I embed the form the big REPORT ABUSE button is stretched out the width of the website. It's ugly and confusing. JotForm and a host of other software form providers don't have that, so not sure why you need it. Also, what type of abuse are they reporting? They are voluntarily filling out a form? Is there anyway to remove that? I think you need to switch your legal counsel. Any plans to upgrade the look?

We respect your opinion. In case you find AidaForm unattractive it's your right to use any other alternatives. We believe it is just a matter of taste.
Regarding the "Report Abuse" button we would gladly remove it from your forms after signing a proper agreement covering legal issues and responsibilities.
The abuse report is developed to give people an opportunity to report to us illegal activities. Not all the people who fill in forms realize that the forms may be fraudulent, and not all form publishers use them for a good use.
We have faced multiple cases when our forms were used for illegal activities such as phishing and race discrimination.
