As for three codes, you can buy three codes, but you can stack a maximum of 2 codes. The additional code is there as an option if you want to setup multiple team accounts or want to gift someone a code ;)
I confirmed the three-code thing with the AppSumo team.

In one of your comments you mentioned it's $10/1TB. Is that one-time or a monthly fee?
Extra storage is $10/TB/Month. You are only billed for storage you use in a given month.
So if you used just 100GB over the limit, you would be charged for just that.
