Q: User for waivers for exercise facility?
Hello, I have a exercise facility that requires visitors to fill out a waiver and submit it. They currently do it all manually. Is there a way to have this hosted on the client website, and then the visitor to the facility can go online, enter in their information into the waiver (name/DOB/email/address/phone/parent info if applicable), and sign the waiver and the owner of the facility gets notified and the completed waiver is sent to the person who filled it out?

You can also provide instructions on what to do after document generation or redirect users to specific URLs after form completion (please keep in mind that Documentero does not offer digital signatures).
