1) Yes.
2) Yes, the plugin creates an individual database for each subsite. However, the WordPress Users list on each website will include ALL Users on the MultiSite install. That's the real problem. And that's why we don't officially support MS although it works.
3) No. You can simply activate your license for the domain of the primary site of your Multisite install in your account manager. Then, all your current and future MailPoet enabled sites will be Premium, regardless of the domain management: subdomains, subfolders or domain mapping.
