It's not clear if once I design a theme, can I easily apply it to 100 sites at once. Or do I need to do the work one by one?

If your 100's sites are different domain then you need to work individually all of them, you can export/import settings from one to others.
If your 100 sites on Multisite then you can clone settings from one to all of them.
