I was having yearly plan of flexi funnnels. If I purchase this LTD, can I activate it using the old ( previously used) mail id ?
also, what is the difference between tier 5 LTD and premium plan mentioned in the site?

No, you can't activate the AppSumo LTD on the same email ID that has your yearly plan. You'll need to use a different email to redeem the LTD code.
