I am very excited about Formaloo seems to have exactly what I need.
I am a one person operation in the health and wellness field. So it is all on me and I am not very tech educated (no idea what all the tech acronyms mean) but I can find my way around a computer..... I have been looking for a solution like Formaloo for a long time and I feel a huge amount of frustration has lifted. I almost didn't try it due to some of the reviews (and honestly the intro video tells you nothing about what this truly is)...But I am glad I did...and I am not finding the same problems other reviewers had....like the look of the templates (very clean and professional) or the help on how to use this software....there are tons of articles and video's that show exactly what to do....Here is what I needed in a form software and couldn't find....now Formaloo is providing all of it:
Clean professional looking forms
Many different answer types and responses
Ability to have multiple questions on one page, or have one question per page
Form logic...so I can ask one question and depending on the response, can follow it up with another.
Ability to auto send response to the client
Alerts that someone has filled out a survey - THIS IS HUGE....I was missing a lot of responses and had to comb individual emails in my CRM each day to see if someone responded.
Ability to have a responsive email sent to someone if they answer a question a certain way....saving me from having to constantly monitor responses....now my client is immediately offered a solution by a trigger
Improvements I would love to see:
Sometimes the platform is slow to load pages or loads and reloads. It especially reloads constantly if you change the screen size (say half your desktop window) this really gets bad and you can't work on your survey.
The ability to switch the order of questions on a page
The ability to drag a question from one page to another.

Right now, you need to know exactly the way you want to put the survey together and if you want to insert a new page of questions between the ones you already have, you can't...you have to create a new page, rewrite all the questions on it to clear the page above and then write the new questions into the cleared page.
Update 12/14/25: There is the ability to switch the order of questions and to drag a question to a different page.

Also I have good news, we do have ability to switch the order of questions on a page and ability to drag a question from one page to another!
In your form editor, on the sidebar on the left, just drag and drop the questions to your desired section and you're good to go!

Good Utility, Some Usability Issues
Like others, I’ve bought a lot of form creation tools. I think I bought this one at the same time as I bought others from App Sumo. Collecting data is important in my line of work.
I left Formaloo in my library for a while. But maybe 6 months ago, we needed a tool that would capture UTM parameters from postcard marketing campaigns and advisor / client actions. Formaloo is the one tool that enabled us to do this - although making it work wasn’t intuitive. Lot’s of trial and error - aided by a stubborn streak - eventually produced what we needed. After I figured it out, this tool became a great value add to our business. We use Formaloo paired with Zapier to process inquiries, onboard clients (set up our CRM, send NDA’s, initiate sequences, etc). Formaloo is featured prominently in our workflow today - mostly because it could manage this one feature that many others could not.
On the down side:
1. the transition from 2.0 to 3.0 is a little clunky. I have forms in both versions and dread the day 2.0 gets turned off. Fingers crossed they convert old forms and data without much friction and no loss of data.
2. It is not intuitive to do what we did with UTM parameters and I could not find support to help me. I did not easily understand the differences between using a regular data field - set to admin (hidden) and configured with a defined field ID, for example, or the use of the “variable” objects. It took a lot of trial and error to get it right. Variable objects are the way to go for this use case, by the way.
3. Don’t try to embed if you’re trying to capture UTM parameters passed by scanning a QR code. The embed structure essentially firewalls the UTM arguments from passing into the form - so you get nothing. In this instance, Formaloo allowed us - with some effort - to create a form that looks professional enough to directly face clients. Most don’t notice they are not in our system when they scan the QR code and get their engagement started via our Formaloo form.
4. Finally, Formaloo automatically sorts responses from first to last. You can switch the sort, but in my instance, it reverted back on it’s own. After a page full of responses, our Zapier Zap triggered by “New Response” failed. It kept returning the first entry - which failed the Zap logic and went nowhere. We adjusted by using a “catch hook” trigger and everything works fine now.
I’m grateful that anything worked at all, so props to Formaloo for having a better technical structure than some of their competitors. Just have patience and persistence if you want to optimize it’s capabilities.

Niche Features Shine, But Form Building and Support Falls Short
I was very excited when Formaloo returned to AppSumo. It always looked shiny and powerful, with capabilities that seemed to position it as a serious contender. I initially bought Tier 3, refunded it within 3 days out of frustration, but returned for Tier 4 because of genuinely unique features I couldn't find elsewhere:
What Actually Works:
- Sequential Multi-Signature Workflows: Multiple people can sign the same form sequentially with "Save & Resume Later"—rare and valuable for approval chains like fire association permits or construction sign-offs
- Dynamic API-Powered Dropdowns: Pull live data from external APIs or internal tables for real-time sync without manual updates
- Advanced Logic & Calculations: Unlimited conditional jumps and real-time calculators for sophisticated, adaptive workflows
- White-Label Portals: Custom domains with branded client portals and role-based access
- Enterprise-Grade Security: AES-256 encryption with POPIA/GDPR compliance for sensitive data
- Relational Tables & Automations: Emulate complex workflows via rollups
These features are genuinely impressive and the only reason I'm still testing this.
The Problems:
Here's the irony: Formaloo markets itself as a form builder, but the actual form-building experience is surprisingly weak. I've tested virtually every form builder on AppSumo (FormDesigner, FormNX, Formly, Zenforms, Formrobin), and most have more intuitive, better-styled builders.
- Form styling is clunky: Making forms look professional requires far more effort than it should. Adding a logo or title image is awkward and unpolished—other builders make this effortless
- "Logic" that isn't logical: I wasted hours troubleshooting a simple conditional flow that refused to work—turned out to be a bug. The form logic system is unreliable and needs significant refinement
- YouTube tutorials are marketing, not education: When you're stuck, these "tutorials" don't help—they're product demos
- Formaloo tries to be Airtable with forms: It ends up being mediocre at both instead of excelling at either
The Deal-Breaker:
There's no direct support contact. The "support" link leads to help pages and tutorials—not actual technical assistance. For a paid lifetime deal, this is alarming. What happens if my account breaks? Who do I contact? This question was raised in the AppSumo Q&A and ignored. This lack of accountability is unacceptable for a professional tool.
Bottom Line:
The niche features (multi-signature, API dropdowns, white-label portals) are powerful and rare—that's what earns it 2 tacos instead of 1. But if you primarily need a reliable, polished form builder, FormDesigner or FormNX will serve you far better with less frustration.
I'll continue testing through the 60-day window, but only because those niche features solve specific problems I can't solve elsewhere. If Formaloo adds real support and fixes the core form-building experience, this could be 4-5 tacos. Right now, it's a tool with impressive capabilities buried under a frustrating user experience.
Who this is for: Users with very specific needs (sequential signatures, API integrations, white-label portals) who can tolerate rough edges and zero support. Everyone else should look elsewhere.

Tier 4 buyer road tested. Not great
a frustrating tool:
- you cannot have a title on each page and make it big and stop the forms fields from appearing next to it. im using two column layout so left column a small title shows, right column shows a form field, looks awful.
- simple things like changing font size and weight is done by css only.
- same with having form fields side by side, also done by css only.
- cannot put a border around a form field.
- cannot show uploaded images from a webform in outputted pdfs
- forms themselves are almost page size, another annoyance and more css
bugs: form fields, right now i add placeholder text to one form field, and it changes all the other form fields placeholders.
its the UI build and design element that lets this down so much. make a proper tools and column controls. see opnform, they do this so well. I can toggle this is a form that i want to charge for - the rest of the settings have zero logic - I mean literally blank and no help.
support is hit and miss, mostly miss. no point having live-chat if it takes hours to respond. at least add a trained ai to answer 'how to' questions and report bugs.
probably going to refund. its that frustrating.
definitely buy this if you LOVE taking screenshots and opening many support tickets and enjoy waiting hours and hours for livechat responses, when everybody appears to be online but do not respond in a timely manner.
this is really not a tool to quick build forms with. Spent a whole day trying to build a 5 page form and its the most frustrating tool i have ever used.

One of the best deal that I used over years
I bought formaloo when it was launched few years back. It was pretty solid at that time and have used it for real projects for lead gen and contact forms and it is still doing fantastic. Over the years, it matured with new versions and new features. If you think you'll ever need form, this one is more than sufficient. And it's one of the deal that I kept using over years.
