Hi Formly team 👋 I want to use Formly on my WordPress site for mental-health assessments (PHQ-9, GAD-7, etc). Can you confirm if it supports:
Conditional logic / branching

Hi, I am testing your platform to create a psychometric test via free plan. I need help with two calculations:
(1) Reverse Scoring: After creating a custom variable say "Open_N1r", how do I assign a calculated value to a custom variable? For example, I need to set Open_N1r = 6 - {Open1}, where {Open1} is the response from a Likert scale question.
(2) Calculating Averages: What is the formula or...

A: Please see our response below: 1. You can create holding variable and use it to perform calculations in "Calculator". BMI calculations example - https://getformly.app/m70LXr Or you can also use "Score Quiz" and assign points/variables directly (e.g. if they select 1 on Likert scale then Add 6, if they select 5 then subtract 2 etc)

Thank you for the clarification. Although I am still not clear on how to assign a score to custom variable which takes an average of multiple questions. Could you also clarify if pdf with charts will be available under all plan? Thanks.
To assign a score to custom variable you can use "Score Quiz" and then use "Calculator" for calculating average etc. Pdf with charts will be available under all Appsumo plan but currently we don't have any specific release dates for this.
I would like to know if I can create a free account on formly and still be connected to Appsumo when I upgrade to $99? AND When I want to upgrade from $99 to Next Plan $198 do I Pay the difference of full amount?

A: 1. Yes, you can create a free account on formly and still be connected to Appsumo when you upgrade to $99 2. When you want to upgrade from $99 to Next Plan $198, you only need to buy one additional code ($99) and you will be upgraded to next plan (Plan 2/Tier 2).

Edited Nov 1, 2025A: Yes, you can collect one-off payment using "Payment" question type (Stripe) but it is not possible to collect recurring payments for subscriptions.
