I've tried messaging your support team but it's been over 24 hours and I haven't heard back yet and I need a quick answer please. My goal is to create forms and use a custom-link with my domain in it, no embedding in my situation.

Can you please let me know if this Custom Domain add-on for Tier 3 is a one-time purchase, or is it something I have to pay for monthly? Can you please specify the price?

Does Tier 4 or Tier 5 get me the custom domain without having to pay extra?

Lastly, please, what does the 1 included CNAME provide us with, since its not considered a 'custom domain' for the purpose of sharing the links to my forms? For example: forms.mydomain.com - What is the free/included CNAME doing for us otherwise, please?
