We have previously used a combo of Wufoo for some things and Google Forms for others. Google Forms is so rudimentary and embedding options are terrible. Wufoo is better, but the ux is terrible and the pricing is through the roof.
GoZen looks great, but is pretty simple to use. Haven't found any bugs yet and looks like this is a keeper.
