Could it be that the OrderForm.com founder forgot to renew their domain? This is a troubling situation as there is complete radio silence. There needs to be some communication.
Q: For the customer's address in the US, the state field always defaults to California which is problematic.
The main issue is that the user may accidentally leave the state field as California which means we have to manually check the address to verify its accuracy each time. This adds a level of uncertainty and additional work on our part which could be easily eliminated if the state field wasn't automatically defaulted to this.
Also, to make things more confusing, if the zip is not entered, there is no message to inform the user that it needs to be filled out (unlike the other fields).
To address these issues, please add the necessary message to inform the user about the zip code when it's not entered. Also, instead of always defaulting to California each time, can you please just add "State" as a light gray label inside it, similar to how "City" and "Zip" are shown inside those fields? Thank you.
Additionally, it would be helpful if there was a verification method built into the payment form to confirm that the entered state and zip code info match. Without this information, it just creates problems with shipping accuracy.
Q: Only one template (one with the color background) shows the phone number field when the option is enabled.
Thanks. There is also some odd spacing between Addresses 1 and 2 and City and State in the form without the phone number. Hopefully, you can make those more consistent.
After the update, the phone number field was removed from previous forms that used it so I had to add this back in. You may want to ensure that this doesn't impact people's current forms. Also, when using the drag/drop option for the phone number field in the form with the color background, it doesn't add the heading above it like the others, so it has to be added manually which feels unnecessary.

I noticed the missing phone number fields with my forms as well. Having to add these back in is disappointing!
