The custom forms appear to be nicely implemented; however, I'd like the ability to:
1. Create and capture a custom field attribute during the Profile setup (i.e. Chapter via dropdown list)
2. On Custom Form 1, have the Chapter field pre-populated based on the value entered in the user profile, capture form field 2, and form field 3.
3. Lastly, I'd like to create an app page that contains a report of all data entered for a specified Chapter.

Is this possible? It seems as though there's a datagrid widget/feature that's missing. Otherwise, it's looking okay so far.
