We were looking for a solution to design nice forms to send via email to our clients in order to get information through surveys. GoZen forms it's a good solution but it's not 100% perfect, I hope it will be not far. The colleccted data cab be downloades as a CSV and you can import it to Excel to create and design your reports. I'll be waiting next improvements of this tool
